If you're coming from Tumalo, Redmond, or other areas north of Bend, take US-97 South toward Bend. Turn right onto SE Truman Ave, then another right onto SW Hill St. Turn left onto Wilson Ave and take the second exit at the roundabout. Continue straight, then turn right onto SW Bluff Dr. Finally, turn right into the Century Plaza parking lot. Our clinic is located in the Millview Building at the back of the lot. Look for a green building with brick accents near the loop in the parking area.From the South (Sunriver, La Pine, or Beyond)
For those traveling from Sunriver, La Pine, or other areas south of Bend, take US-97 North toward Bend. Take Exit 138 for Reed Market Rd, then turn left onto SW Reed Market Ave. At the first roundabout, take the first right onto Bond Street. Turn left onto Bluff Dr., then right into the Century Plaza parking lot. Our clinic is located in the Millview Building at the back of the lot. Our main entrance faces Bluff Dr. Look for the door marked with the Aspire Physical Therapy logo. Once inside, take the stairs just past the entrance, or use the elevator for easy access.

All appointments, including your initial evaluation and follow-up treatments, are one hour long and conducted one-on-one with a licensed provider to ensure personalized care and attention.

Yes, completing your paperwork ahead of time will speed up the check-in process. If you are unable to do so beforehand, please arrive 30 minutes early to complete it before your session.

Wear athletic or stretchy clothing: Opt for breathable fabrics like leggings, sweatpants, shorts, or a t-shirt to move freely.
Choose supportive footwear: Sneakers or athletic shoes with good grip and support are ideal. Avoid sandals, flip-flops, or dress shoes.
Dress for your treatment area: If you’re receiving therapy for your knee or leg, wear shorts. For shoulder or upper body therapy, a tank top or loose-fitting shirt works best.

If your insurance plan is not listed, you may still receive care with our discounted cash pay rate, and we can provide a superbill upon request. This allows you to submit the superbill to your insurance for potential reimbursement under out-of-network benefits.

Yes! Aspire Physical Therapy accepts Motor Vehicle Accident (MVA) claims and all workers' compensation cases. If your injury is related to an auto accident or workplace incident, please inform us when scheduling your appointment.
Motor Vehicle Accidents (MVA):
We require your claim number, policy number, auto insurance company, and adjuster’s name before your first visit.
If you don’t have a claim open yet, we can assist you in starting the process.
We bill the auto insurance company directly, but MVA claim processes may vary by state.
Workers' Compensation Cases:
We accept all workers' compensation claims for work-related injuries.
Please bring your case number and adjuster’s name to your first appointment to ensure smooth processing.

The need for a referral depends on your insurance plan:
Medicaid or Medicare: A referral from your doctor is required before beginning treatment.
Commercial Insurance: A referral is not necessary, but we’re happy to accept one if you have it.

Co-pays, co-insurance, and deductible payments are due at the time of service.
We accept a variety of insurance plans—please contact our office if you have any questions about coverage.
If your insurance requires prior authorization, please let us know so we can submit the request after your first visit is completed.
For cash-pay patients, we offer self-pay rates—please reach out to our office for details.

Item descriptionTo ensure we can provide timely care to all patients, we have a 24-hour cancellation policy.
No-Shows & Late Cancellations: A $60 fee will be charged for missed appointments or cancellations made with less than 24 hours' notice.
Monday Appointments: If you need to cancel a Monday appointment, please leave a voicemail or send a text over the weekend. Cancellations made on Monday will be considered late and subject to the $60 fee.
Late Arrivals: If you arrive more than 10 minutes late, your appointment will be considered a no-show, and you will be charged the $60 fee. You will also need to reschedule.
